Lasting Wishes helps families record a loved one's personal wishes through a private, hosted questionnaire. This policy explains, in plain language, what we collect, where it is stored, and how you stay in control.
The questionnaire asks you not to enter account numbers, passwords, PINs, or other login credentials. Please do not enter them. A location or a description is enough.
Questionnaire text is stored in Cloudflare D1. Photos and generated documents are stored in Cloudflare R2. Purchase and contact records are stored in our customer system and payment processor. Each questionnaire is reached only through a private, unguessable link.
We use this information only to create and deliver your personalized questionnaire and the resulting summary document, and to support you if you contact us.
We do not sell your information. We do not share your answers with any insurance agent or other third party for their own marketing. Your answers are not advertised or published.
The person who completes the questionnaire controls their answers. Nothing is shared until they reach the end and choose to finish.
We keep your information for as long as your account is active or as needed to provide the product. You can request a copy or deletion of your data at any time by emailing privacy@lastingwishes.co. We will confirm and complete deletion requests within a reasonable time.
We take reasonable steps to protect your information, including private links, encrypted connections, and access controls. No online service can promise perfect security, so we do not claim it.
