Lasting Wishes captures personal preferences, stories, and messages through a private questionnaire and produces a summary document for your family. It is a keepsake and a record of wishes.

This product is not legal, financial, or medical advice, and we are not your attorney or advisor. It does not create a will, advance directive, power of attorney, or any binding legal instrument.
This document records personal preferences as of the date shown. It is not a will, advance directive, power of attorney, or insurance policy. Where legal documents exist, those documents govern.
Your responsibilities
- Provide accurate information and use the product for its intended, lawful purpose.
- Do not enter account numbers, passwords, or other credentials into the questionnaire.
- Keep your private link private. Anyone with the link can view the questionnaire it points to.
